| DECLARATION AND COVENANTS |

| I hereby confirm, declare and agree as follows : |

| 1. | that neither me nor my spouse (if married) is an existing distributor or ex-distributor of the Company; |
| |
| 2. | that
I will present the Company's products and BDP to potential distributors
and customers in a truthful and honest manner and will not make any
false or misleading representation in my enthusiasm to sell the
Company's products or to recruit or sponsor potential distributors; |
| |
| 3. |
that I am aware that my appointment as a distributor is valid for a
period of 12 months unless earlier terminated by the Company. I am
fully aware that I have to apply to renew my distributorship at least 3
months before its expiry; |

| 4. | that
I shall comply with all terms and conditions set out herein and in the
Distributors Rules and Regulations, the BDP, the Direct Sales Act 1993,
the Direct Sales Regulations 1993 and all laws and regulations relating
to direct sales as well as the Terms and Conditions set out in this
Site. I am fully aware and agree that the Company may amend the
Distributors Rules and Regulations and the BDP at any time and from
time to time as the Company deems fit; |

| 5. | that
I am an independent contractor and is not an agent or employee of the
Company and is a Malaysian citizen and is above the age of 18; |

| 6. | that
I wish to subscribe to CNI News and hereby authorise the Company to
deduct the subscription fee for CNI News from the bonus payments due to
me. Should I wish to discontinue the subscription for CNI News I shall
notify the Company in writing of such intention at least 3 months
before the expiry of the term of my distributorship. I further agree
that no refund would be entertained once a deduction is made; |

| 7. | that all the information given in the above Registration Form are true, accurate and correct; |
| |
| 8. | that
the Company shall be entitled to revoke or terminate my distributorship
at any time in the event any information given by me is false,
inaccurate or misleading . |

If my application is not approved, I agree that you shall be entitled to deduct a sum of RM10.00 as administrative fees |
